Summit fees include: Conference keynote and breakout sessions, sponsor receptions, concierge service, Summit materials, industry exhibits and pre conference workshops. Resort fees include accommodations along all meals, beginning with dinner June 28 through lunch Tuesday, June 30. Welcome Reception is Sunday at 6:30pm. Attendees who are not staying in the hotel will be charges a $198 resort fee for the conference.

HOTEL RESERVATIONS
Hotel reservations are available at the Eaglewood Resort & Spa through PDW Concierge. The rate of $198 + tax per night includes all meals beginning on Sunday Night, June 28 through lunch on June 30th. Call Judy Neustadter at PDW at 914 232 1747. Please make room reservations early. DO NOT MAKE RESERVATIONS DIRECTLY WITH THE EAGLEWOOD RESORT & SPA.

ABOUT THE RESORT

TRANSPORTATION
Airport transportation is available through A1 Limousine - they offer a shared-ride service for only $22 per person, one way. Reservations are required in advance. Call 630 833 3788. Once you have landed at O'Hare and you have retrieved your luggage from baggage claim - proceed to the nearby pay phones and dial 1 800 354 5849 or by cell call 630 833 3788.The dispatcher will provide you with a license plate # and you will walk outside to the second curb and the car will be there within 10-15 minutes...sometimes sooner. For your return to O'Hare from Eaglewood - at time of reservation they will provide you with a pickup time - they will pick you up at Eaglewood main entrance.

CANCELLATION POLICY
Cancellations must be submitted before June 1, and will be refunded less a $100 administrative fee. Substitutions may be made at anytime without charge.
